Effective May 1, 2026, The process for submitting many required forms will change to an electronic format.
Please note that the process to report lost or stolen controlled substances will also change on May 1, 2026. Pharmacies will report only after confirming that a loss has occurred. It will be a single report to replace the current two-step reporting system.
The Reporting Requirement Overview on the Reporting Forms page will always have the most updated submission information for mandated reports.

Beginning on August 23,2023 licenses issued by the Board of Registration in Pharmacy will transition to a new online licensing system
